Analysis of the incidence of mental and behavioral disorders associated with the use of psychoactive substances among the male population of the Tomsk Region over a 10-year period

Abstract
Among common socially significant diseases, mental and behavioral disorders due to psychoactive substance use remain one of the most acute medical and social problems of modern times, especially among the male population, requiring a comprehensive decision. Systematization of regional epidemiological monitoring data makes it possible to assess the effectiveness of preventive, curative-rehabilitation programs, to identify local trends and structural shifts in morbidity, as well as to justify targeted strategies for medical and social intervention. Objective. To analyze the dynamics of prevalence and primary incidence of mental and behavioral disorders due to psychoactive substance use among the male population of the Tomsk Region over a 10-year period (2015–2024). Material and Methods. A retrospective analysis of data from federal statistical reporting form no. 11, “Information on substance-related disorders”, was conducted. The following categories were investigated: alcohol dependence syndrome (ADS), alcoholic psychoses (AP), harmful alcohol use (HAU), drug dependence syndrome (DDS), and harmful drug use (HDU). The indicators were calculated per 100,000 of the average annual male population. Average annual growth rates and the range of the dynamic series were used. Results. Primary incidence decreased by 71.6% ‒ from 388.8 to 110.4 per 100,000 of the male population, the average annual decline rate was 13.0%. The largest reduction was recorded for HAU (86.8%), HDU (89.8%), and AP (71.5%). The ADS indicator decreased by 33.3%, and the DDS ‒ by 60.2%. The number of injection drug users (IDU) decreased by 74.9% (5-fold decrease). The structure of alcohol-related disorders has worsened: the percentage of ADS increased from 25.1% to 56.7%, the percentage of HAU decreased from 56.6% to 25.6%. In the dynamics of drug use disorders, we observed the opposite: the percentage of HDU increased from 16.1% to 57.1%, the percentage of DDS decreased from 83.9% to 42.9%. The overall incidence decreased by 15.7%, from 2012.9 to 1696.5 per 100,000. The largest decrease was recorded in the overall incidence: AP (60.7%), HDU (35.1%), and HAU (19.2%). Conclusions. In Tomsk Region, a steady decrease in the incidence of substance use disorders was noted in 2015–2024. Simultaneously, differently directed structural shifts occurred: an increase in the severity of alcohol pathology and a shift in the structure of drug use disorders toward less severe forms. The results show the effectiveness of preventive measures, but require continued attention to the treatment of chronic forms of dependence.
