Evaluation of the effectiveness of combination therapy using SSRIs and bioacoustic correction in patients with panic and generalized anxiety disorders: focus on somatovegetative disorders

Abstract
Materials. The study was conducted at the Department of the Treatment of Borderline Mental Disorders and Psychotherapy of the Bekhterev National Medical Research Center for Psychiatry and Neurology from February 2024 to February 2025. Objective: comparative assessment of symptom dynamics in patients with panic disorder (PD) and generalized anxiety disorder (GAD) receiving SSRI pharmacotherapy and its combination with bioacoustic correction (BAC). The clinical sample included patients (n=60) with PD and GAD, divided into two groups: the main group (n=30) ‒ receiving medications, 15 BAC sessions; control group (n=30) ‒ receiving only medications (sertraline, paroxetine, fluvoxamine, fluoxetine). Research Question: to evaluate the effect of SSRI pharmacotherapy and BAC on the severity of anxiety-depressive manifestations, asthenovegetative and sleep disorders in patients with PD and GAD with subclinical depressive symptoms. For the purpose of psychodiagnostic assessment, specialist tests, scales and questionnaires were used: HARS (anxiety), PHQ-9 (depression), the Wein questionnaire (vegetative disorders), ISI (insomnia) and CGI (clinical state). Results. In the main group, the combination therapy demonstrated a significant decrease in anxiety (HARS: from 26.03 to 14.16 points) and depression (PHQ-9: from 14.37 to 4.18), improvement in autonomic parameters (statistically significant decrease in autonomic disorders according to the Wein questionnaire by 34.5% ‒ from 35.20±12.66 to 23.06±12.52, p<0.001) and sleep quality (ISI: from 15.00 to 8.46). In the control group using only psychopharmacotherapy, a less pronounced improvement was found (HARS: from 22.33 to 17.63; PHQ-9: from 13.10 to 9.43). Intergroup analysis revealed statistically significant advantages of combination therapy, especially in the correction of emotional symptoms. BAC enhances the effect of pharmacotherapy, effectively reducing anxiety-depressive and vegetative symptoms, as well as improving the sleep. Method is safe and recommended to patients with resistance to standard treatment.
