Influence of tobacco smoking on the development of mental disorders in patients with tuberculosis

Abstract
Introduction. Tuberculosis is often combined with comorbid mental disorders (anxiety, depression) and nicotine addiction (smoking), which have a mutual negative impact on the effectiveness of therapy. Comorbid mental disorders further aggravate separation, distancing and stigmatization of tuberculosis patients. The negative impact of tobacco smoking on adherence to treatment in tuberculosis patients has been proven, which leads to a decrease in its effectiveness, the formation of chronic forms of tuberculosis, and frequent relapse of the disease. Objective: to assess the impact of tobacco smoking on the development and aggravation of anxiety and depressive symptoms in tuberculosis patients. Materials and Methods. The clinical retrospective study included 265 patients who were treated in the clinical departments of the Novosibirsk Research Institute of Tuberculosis of the Ministry of Health of the Russian Federation in 2021-2023. All patients underwent a comprehensive clinical and radiological examination of the chest organs. When assessing the mental status of patients, the HADS scale was used to identify clinically significant anxiety and depression. Results. The proportion of smokers among patients in the study sample was 65.3%. Smoking in these patients was accompanied by an increase in the number of both subjective symptoms of the disease (cough, shortness of breath, symptoms of intoxication) and objective symptoms (increased levels of C-reactive protein, the area of decay in the lungs, which positively correlated with the severity of depression and had an inverse relationship with the severity of anxiety). Conclusion. The existing complex relationships between smoking, clinical symptoms of the tuberculosis process and psychopathological (anxiety and depressive) symptoms require a multidisciplinary approach using drug and non-drug therapeutic means.
