Abstract

Background. Radical changes in modern society are due to the high prevalence of various types of phobias (social phobia, agoraphobia), developing against the background of stress of varying intensity. The rapid increase in the number of patients with social phobias, including those with comorbid mental disorders, reflects the significance of this problem in psychiatric practice. Objective: to identify the structure of personality pathology comorbid with social phobia, comparative analysis of correlations between personal characteristics and quality of life parameters of patients with social phobia, comorbid with personality disorders and other mental disorders, to clarify the etiology and pathogenesis, determine clinical and diagnostic differences in comorbid diseases with subsequent planning of therapeutic approaches. Material. The study sample (n=135) consisted of patients aged 18-47 years with the diagnosis of non-psychotic mental disorders according to ICD-10, with an average duration of social phobia of 13.1±8.1 years. Methods: psychodiagnostics testing: 1) sMMPI ‒ a standardized multifactorial method of personality research; 2) Russian version of the general health questionnaire MOS SF-36. Statistical data processing was carried out using STATISTICA version 10.0 with nonparametric methods. Results. Comorbid personality pathology was expressed by mixed personality disorders (n=18; 13.3%). The prevalence of comorbidity of social phobia with other mental disorders was revealed (n=117; 86.7%): mixed anxiety-depressive (n=27; 20.0%), adjustment disorder (n=21; 15.6%), somatoform dysfunction of the autonomic nervous system (n=18; 13.3%), dysthymia (n=14; 10.4%), panic (n=11; 8.1%), recurrent depression (n=8; 5.9%), obsessive-compulsive (n=7; 5.2%), generalized anxiety (n=6; 4.4%), bipolar (n=3; 2.2%), mild depressive episode (n=2; 1.6%). For all types of personality disorders comorbid with social phobia, common characterological features have been established in the form of social avoidance associated with unstable self-esteem and high self-esteem instability, increased sensitivity to external evaluative signals and uncertainty in self-acceptance The leading features of the personality profile of patients with social phobia, comorbid with personality disorders, were identified: high values on the scales “Hypochondriasis”, “Depression”, “Psychiatric Asthenia”, “Schizoid Personality” and low values on the scale “Hypomania”. The specificity of correlations between quality of life parameters and personality profile scales has been established, depending on the presence or absence of comorbid personality pathology. The findings can be used in the differential diagnosis of social phobia and will allow for pharmacotherapy and psychotherapy taking into account the identified disorders.
