Abstract

Background.The use of hospital-substituting technologies is aimed at reducing hospitalizations in 24-hour psychiatric hospitals, optimizing social functioning and reducing the stigmatization of psychiatric patients. Objective:based on monitoring of the main indicators of the work of a hospital at home and a day hospital at an adult dispensary, to evaluate the effectiveness of the hospital-substituting technologies used, to develop proposals for optimizing measures of the medico-social rehabilitation strategy. Materials. The study, carried out based on a hospital at home and a day hospital at the adult dispensary department of Tomsk Clinical Psychiatric Hospital, is focused on the further development and optimization of hospital-substituting technologies. Based on statistical reporting information, an analysis of the main indicators (number of patients: admitted, discharged, used, average length of stay in a bed, bed turnover) of the treatment and preventive activities of these units providing care in an outpatient setting has been carried out. Methods:statistical, comparative, analytical. The condition of the treated patients has been assessed according to the diagnostic criteria of ICD-10. Discussion. A study has been conducted on the problem of organizing psychiatric care for patients in a psychiatric hospital for 2020-2022, based on Tomsk Clinical Psychiatric Hospital. A historical overview of the development of hospital-substituting services in the Tomsk Region is presented. An assessment of the final annual indicators that make up a comprehensive characteristic of the effectiveness of the organization of out-of-hospital care is given. Proposals are developed to optimize measures of the medical and social rehabilitation strategy, directions for the development of inpatient substituting services are determined in the structure of the Regional State Autonomous Institution “TCPH”.
