Особенности использования электросудорожной терапии в психиатрической практике в различных регионах мира (обзор литературы)

Реферат
Актуальность. Использование ЭСТ демонстрирует различия в разных странах мира, однако всесторонний, основанный на доказательствах практико-аналитический обзор реализации ЭСТ не проводился. Цель:охарактеризовать текущие модели использования ЭСТ в 6 регионах ВОЗ (Африка, Северная и Южная Америка, Юго-Восточная Азия, Европа, Восточное Средиземноморье и Западная часть Тихого океана). Материалы и методы. Поиск литературных источников в основном проводился через базу данных PubMedи по материалам журнала “TheJournalofECT”, которые являются главной платформой для сбора актуальной, основанной на доказательствах информации о практике применения и научных исследованиях ЭСТ. Результаты. Максимальные масштабы использования ЭСТ в мире наблюдаются в Северной Европе, где превалируют новейшие технологии, полностью соответствующие строгим, официально утвержденным стандартам её проведения. В то же время в Западной и Южной Европе есть страны (Люксембург, Словения), в которых ЭСТ не входит в перечень разрешённых процедур (находится под запретом). В среде пользователей ЭСТ на всех территориях ВОЗ преобладают женщины в возрасте 18-59 лет, страдающие аффективными расстройствами (депрессией, манией), кататонией и психозами (шизофренией). Однако стратегия выбора терапии с включением ЭСТ очень вариабельна – от 1-й до 5-й линии (шага) в терапии той или иной психической патологии (состояния). В мировой практике используется битемпоральное наложение электродов устройств с синусоидальными волнами и/или (ультра)короткими импульсами, только в Северной и Западной Европе, США, Австралии, Новой Зеландии – униполярное наложение на правое полушарие. Лекарственное пособие чаще всего включает общую неингаляционную анестезию метогекситалом (Methohexital), пропофолом (Propofol), этомидатом (Etomidate) и миорелаксантом ультракороткого действия суксаметоний (succinylcholine). Обобщенные результаты метаанализов и систематических обзоров показывают, что ЭСТ обеспечивает высокую частоту терапевтического ответа (>60%) и ремиссий (>40%), приводит к сокращению числа госпитализаций, снижению риска самоубийств, улучшению функциональных возможностей и качества жизни больных. Заключение. Вопреки имеющимся территориальным отличиям и региональным особенностям в доступности ЭСТ, повсеместно отмечаются признаки стандартизации технологических процедур выполнения. Более широкое её применение наблюдается в странах с низким уровнем доходов населения. Существенные территориальные контрасты отмечаются даже в экономически процветающих государствах. Имеющиеся данные преимущественно поступают из стран Глобального Севера с высокими уровнями экономического развития и жизни населения, что ограничивает всестороннюю оценку использования ЭСТ в мировом масштабе.
