Реферат

Актуальность. Коморбидность психических расстройств с зависимостью от психоактивных веществ (ПАВ) и высокая её распространенность у правонарушителей определяют приоритетность дальнейшей научной разработки концепции двойного диагноза в психиатрии, а также особенностей самоконтроля и дисрегуляции поведения у этой категории пациентов. Их изучение чрезвычайно значимо для прогнозирования риска общественной опасности и определения тактики превентивных вмешательств. Целью обзора является систематизация данных о клинических, когнитивных и эмоциональных особенностях пациентов, страдающих психическими расстройствами и коморбидной зависимостью от ПАВ, оказывающих влияние на регуляцию поведения. Материалы и методы. С помощью наукометрических баз данных (PubMed, Embase, ClinicalTrials, GoogleScholardatabases, eLibrary) проведен поиск научной литературы. Отобраны публикации, в которых отражены нарушения доменов саморегуляции (когнитивные и метакогнитивные функции, эмоциональная регуляция) и особенности клинической картины у лиц с психическими расстройствами и коморбидной зависимостью от ПАВ. Результаты. Согласно результатам обсуждаемых исследований, коморбидность оказывает существенное влияние на формирование дисрегуляции психических функций у лиц с тяжелыми психическими расстройствами. Пациенты с так называемым двойным диагнозом имеют более высокую распространенность и выраженность нарушений поведения, таких как импульсивность или снижение контроля, а также значительные когнитивные и метакогнитивные дефициты, представленные дисрегуляцией исполнительного функционирования, нарушениями метакогниций. Специфика их эмоциональных дисфункций нуждается в дальнейшем изучении. В научных работах также содержится указание на важность обучения эмоциональной регуляции при психических расстройствах, коморбидных с зависимостью от ПАВ. Исследователи отмечают влияние эмоциональных реакций, не пропорциональных вызвавшему их раздражителю, и затруднений в управлении эмоциями на клиническую картину в виде её усложнения, в том числе в результате взаимного влияния когнитивных нарушений и расстройств поведения. Заключение. Полученные данные свидетельствуют о необходимости дальнейших исследований для лучшего понимания механизмов взаимодействия клинических особенностей и доменов саморегуляции, их влияния на дисрегуляцию поведения у данной категории пациентов. Выявление специфичных механизмов нарушения регуляции у пациентов с коморбидной зависимостью от ПАВ является значимым стимулом для разработки диагностических и лечебных методов, направленных на улучшение качества жизни и снижение риска общественной опасности.
