Abstract

Introduction. Epilepsy is a common chronic neurological disease associated not only with a certain burden of life, social marginalization and stigma, but also with comorbid mental disorders. Objective: to identify the characteristics of clinical manifestations of suicidal behavior in patients with epilepsy. Materials and Methods. Based on the Department of Treatment of Exogenous Organic Disorders and Epilepsy of V.M. Bekhterev National Medical Research Center of Psychiatry and Neurology, 80 patients with epilepsy aged 18-60 years were examined. Patients were divided into two groups depending on the presence of suicidal ideation at the time of the clinical interview. Group 1 – patients (n=45) with epilepsy without suicidal thoughts, Group 2 – patients (n=35) with epilepsy with passive suicidal tendencies. The study used scales and questionnaires to verify affective disorders and suicide risk (questionnaire for the severity of psychopathological symptoms, the Hospital Anxiety and Depression Scale, the Reasons for Living Inventory, suicide risk questionnaire). Processing of the findings included quantitative and qualitative analysis. Statistical processing of the material was carried out using SPSS.23 and Excel programs. Parametric data were assessed using descriptive statistics methods (calculation of average values, standard deviations, frequency analysis). For parametric data, the statistical significance of differences between the indicators of the study groups was assessed based on the Student’s t-test, for non-parametric data ‒ the Mann-Whitney U-test and Fisher’s φ*-angular transformation. Statistical hypotheses were tested at the level of statistical significance p=0.05. Additionally, correlation analysis was used (Spearman's rank correlation coefficient). Results. Patients with suicidal thoughts showed a lower significance of anti-suicidal motives than patients without suicidal ideation. A statistically significant direct relationship was found between the severity of affective disorders and the degree of suicide risk. Conclusion. Due to the fact, that depression in epilepsy often did not meet the criteria for standard depressive disorders, for its timely detection it was necessary to use more advanced scales and questionnaires, as well as joint supervision of patients both by both neurologists and psychiatrists.
